This is a translated version of the HIPAA Authorization Template, UW and Fred Hutch Cancer Center.
The purpose of this template is to create the HIPAA Authorization form that participants, parents, and/or legally-authorized representatives of participants sign to give you permission to obtain and use protected health information (PHI) of participants for research purposes.
