NIH: FORMS-G
NIH announced the new FORMS-G is required for applications due on or after January 25, 2022.
FORMS-G Highlights:
- Updated Biographical Sketch and Other Support format pages are required for submissions on or after January 25, 2022. See NOT-OD-21-073, NOT-OD-21-110, and NOT-OD-21-122 for more information.
- Transitioning to the new Unique Entity Identification (UEI) number which will replace the DUNS number. See NOT-OD-21-170 for more information.
- The UW UEI is HD1WMN6945W6, also available from our Facts & Rates page
- All Senior/Key personnel listed on the R&R Senior/Key Person Profile (Expanded) form will be required to have an eRA Commons username (Commons ID). See NOT-OD-21-109 for more information.

- Increased “Other” Direct Cost lines from 3 to 10
- PHS Fellowship Supplemental Form
- New childcare cost section
- PHS Human Subjects and Clinical Trials Information
- Study Record change: describe the single IRB plan
- SBIR/STTR Information
- New question 7 on Technical and Business Assistance (TABA)

Application Guide Updates
NIH’s How to: Application Guide will be updated with FORMS-G information no later than October 25th, 2021.
Application Package Availability
For a transition period, both FORMS-F and FORMS-G application packages will be active and available. Applicants must choose the appropriate application package for their due date.
- New funding opportunity announcements (FOAs) posted on or after October 25, 2021 will include the new FORMS-G application package.
- New FOAs posted before October 25, 2021 with initial due dates on or after January 25, 2022 will be posted without application forms until updated forms are available.
- Application packages will be added to these FOAs by November 25, 2021.
- All active Parent and IC-issued FOAs with due dates on or after January 25, 2022 will be updated to add FORMS-G application packages between October 25, 2021 and November 25, 2021.
For due dates occurring on or before January 24th, 2022, continue to use FORMS-F.
NOTE: Building an application package using FORMS-F to respond to an active FOA with a due date on or after January 25th will lead to re-work of the materials in updated FORMS-G. Please consider timing and use FORMs-G for applications due on or after January 25th.

SAGE Grant Runner Applications
For those of you who use Grant Runner to submit NIH proposals, the FORMS-G updates are now available as of Feb. 23, 2022.
NOTE: A Grant Runner application created with FORMS-F cannot be converted to FORMS-G.
UDPATE Feb. 23, 2022: This announcement was originally published on Oct. 1, 2021 and updated again in December and January with a note to use ASSIST until Grant Runner is updated to include FORMS-G. As of Feb. 23, 2022 FORMS-G is available in Grant Runner.
