CHECKLIST Exception from Informed Consent
The purpose of this checklist is to provide support for IRB members by listing the criteria that must be met in order to grant IRB approval for research involving the exception to informed consent, and to document that the criteria have been met. This checklist is a regulatory document that is part of the study file.
