This worksheet is used as a guide by the IRB Chair, IRB Members, and HSD staff to ensure that the appropriate Common Rule Criteria for approval have been considered for items that are being reviewed by the IRB. The UW IRB applies these criteria to all research it reviews except as indicated below. This worksheet is not meant to be completed or retained.
