TEMPLATE Other E-Consent Tool Attestation Letter
The purpose of this form is to assist HSD staff in determining if an ‘other’ system for capturing e-signature meets UW criteria for security and reliability for conducting and documenting electronic consent (including compliance with 21 CFR 11 and with Washington State Law).
Researchers wishing to use an e-signature system other than REDCap, UW Florence eConsent, or UW eSignatures (powered by Docusign) must provide this letter, signed by the appropriate individual.
