Exempt Research Consent Template
HSD does not review or approve consent materials for exempt research but researchers are expected to provide subjects with particular information about the research prior to their enrollment.
HSD provides this template to assist researchers with providing information about the research to subjects, but it is not required that researchers use it. Researchers should not upload consent materials for exempt studies in Zipline.
Researchers may find these example exempt consent document helpful as well.
