This worksheet is for data requests made by shared governance entities to the offices of the Provost and the President. It is not required, but recommended as a best practice to ensure that:

  • Requests are articulated in a clear and concise manner.
  • Requests are aligned with a working priority for shared governance (e.g. a Faculty Council charge, a task force in information-gathering stage, etc.).
  • Executive offices have clear information for intaking, evaluating and making informed decisions about a request.
  • Communication and feedback processes are transparent to everyone involved in the process, increasing collaboration and buy-in to shared governance.[1]
  • If the requesting entity is a Faculty Council, and has a Presidential Designee, the Designee should be consulted on where and how to address the request.
