Child Care Assistance Program Forms

Applications for 2009-2010 are now available. The deadline for applying for 2009-2010 is May 29, 2009 for both full and part-time assistance. Your forms can be dropped off at the Student Parent Resource Center in 172 Schmitz, mailed to Box 355882, or faxed to 206/685-1338.

International students must complete the International Financial Statement in place of the Free Application for Federal Student aid (FAFSA). The International Financial Statement must be turned in with your application for child care assistance.
