Thank you for your interest in the VP for OMA&D/UDO attending your event! Please fill out this form to the best of your ability. After you click submit, copies of your request will be sent to the VP’s office for review. The more complete information you provide, the better our offices can determine if the VP/UDO will be able to attend.
Please submit your requests four (4) weeks or more prior to the event. While we attempt to accommodate all requests, forms submitted less than four weeks may not be approved.
Final confirmation of the VP/UDO attendance varies, but it is our goal to connect with the requestor within seven (7) working days of your submission.
