Several minor updates have been made to HSD forms as listed below:
- Modification Form:
- Training section has been reformatted from flat text to question and answer format. Links to HSD resources have been added.
- Status Report:
- Check box for Approval in Principle has been removed.
- Human Subjects Review Application Instructions:
- Revised to reflect multiple changes to the form itself.
