The Export Control Worksheet prompts for information about items you plan to ship or take with you outside the United States, the end-use of the item, and the end-users. The information you provide helps with export control analysis for applicable regulations and license requirements.
Send completed export control worksheets or questions about the worksheet to exports@uw.edu.
The exports control specialist may provide you with an export control license determination, retain that determination in your records.
