Welcome to the Surgical Critical Care Service at Harborview Medical Center

Over your time here you will manage some of the most complex patients you will ever come across over the course of your career.

These patients often have several services involved in their management.  These patients are usually admitted under the General Surgery, Vascular, or Thoracic Services, however it is the Surgical Critical Care Service that coordinates their care.

Although most of the ICU attendings are surgeons, it is important to note that the scope of the ICU service will be limited to evaluation and management of problems separate and distinct from routine postoperative surgical care.

These services include but are not limited to:

  • Ventilator management
  • Hemodynamic management
  • Management of coagulopathy
  • Diagnosis and management of nosocomial infection
  • Nutritional support
  • Non-operative management of head injury
  • Venous thromboembolism screening, prophylaxis and treatment
  • Management of renal failure exclusive of dialysis
  • Coordination of subspecialty consultations
  • End-of life care

 OVERVIEW

The overall aim of every program is to train you to become a knowledgeable, safe and competent surgeon. These goals – what you are expected to achieve – and objectives – how you are expected to achieve them – are guidelines designed to reflect a series of supervised patient care experiences in a system of graduated responsibility.

Each year at Harborview is designed to present you with a unique set of challenges to help develop your surgical skills in a graduated fashion. Therefore, all seven competency levels in each year must be mastered prior to advancement to the next year of training.

Level-specific competency objectives were prepared by the faculty to give you more concrete examples of the performance standards we expect during each year.

PREPARATION FOR YOUR ROTATION AT HMC

UW Medicine Acct: (Activate your account) This is the first step prior to anything else, and needs to be accomplished prior to your rotation at Harborview. We use your demographic information in MedHub to apply for you. When you receive your confirmation, it is up to you to set up your account. Here are instructions on how to set up a UWnetID and UW medicine account.   Account Info  ** If you are a Program Coordinator from a participating program (Swedish, Virginia Mason, Madigan, etc.) please contact Suzanne Mills for account access issues.

Badge: If you do not have an HMC badge you will not have access to many clinical areas including the OR and Clinic.  You will need to take a signed form to the Badging office on 8CT 69. Bring photo ID. See Ann Haven for Burns and Suzanne Mills for TICU, Trauma Teams 1 and 2 for the form.

ACCESS: to OR, ED, Locker Rooms, Clinics.  This access will be attached to your badge.  Suzanne Mills will request this for you.  In the event of a problem,please email hsurged@uw.edu

PACS: Access to this application allows you to see radiology images and reads, including those imported from outside institutions. Suzanne Mills will apply for access for you, once you activate your UW Medicine account.

OR Whiteboard: Allows you to see the OR schedule for all services in real time.  Suzanne Mills will apply for access for you once you activate your UW Medicine account.

ORCA and EpicCare Training: This training needs to be done BEFORE you start your rotation at HMC. Work with your program coordinator to register. There can be glitches in access to your EpicCare Application, so a best practice would be: If you have not yet been to clinic at Harborview, and know you have completed EpicCare training, you might want to take a moment and test your access to the application.

Navigate to ORCA  Login with your UWNetID and password.
When you come to the main screen,
SELECT EPIC-Hyperspace Then log in again.
If all is configured as it should be for your access, you should be good to go. If at the Epic Hyperspace window you get a message that your role is not configured properly, or something else Is wrong, call the HELP DESK: 543-7012 Doing this will help prevent any “surprises” in clinic and assure you will be able to work with your team starting from the first day of your rotation.

Setting up Preferences in ORCA:  You may not have been able to set up your preferences at training, but will need to do so for your rotation.  ORCA Set up

PAGERS: Those of you coming from Madigan will want to use an HMC loaner pager while here, so number is not long distance.  Bring with you a check made out to the Department of Surgery for $50.00.  We will hold onto the check and return it to you when the pager is returned.  Please contact Suzanne Mills with any questions.

R1

Responsibility focuses on the diagnosis and management of intensive care patients that range from multi-system trauma to post operative multi system organ failure.

The R-2 concentrates on the fundamentals of critical care with a primary responsibility in the intensive care units and in the Harborview Emergency Trauma Cen­ter.

The R-2 performs operative procedures appropriate to that level of training.
