Class description

The Spring 2014 Course section called Care Transitions will prepare students to strategize how to have high quality care transitions for the patients discharging from a variety of health care settings in an effort to prevent unnecessary re-hospitalizations. We will explore the theoretical concepts around care transitions that underpin the strategies used to promote successful transitions from one setting to another. These strategies designed to prevent hospital re-admissions that occur less than 30 days from discharge. Students will study the a variety of topics that may include but are not limited to: Discharge planning, transitional care, Coordination of Care, Models to identify patients at risk for re-hospitalization, Care coordination for Cognitively Impaired older adults, and QAPI strategies to prevent and or identify appropriate hospital re-admissions such as INTERACT 3.0.
