Which version you see depends on whether you have previously disclosed any SFI and whether your sponsor (or originating sponsor) is a one that requires UW review of SFI.<\/p>\n<p>The Disclose Significant Financial Interests\u00a0page appears when you select\u00a0<strong>Complete Disclosure<\/strong>\u00a0or <strong>Create New Disclosure<\/strong>\u00a0in the Action Items section and then select an eGC1 or CoMotion\/IRB activity.<\/p>\n<h2>No Existing SFI<\/h2>\n<p>If you have not yet entered any SFI, you will need to indicate whether you have any SFI to disclose. The following image shows the question.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-1250\" src=\"https:\/\/www.washington.edu\/research\/wp-content\/uploads\/fids_indicate_sfi.jpg\" alt=\"indicate if you have any S F I\" width=\"674\" height=\"298\" \/><\/p>\n<ul>\n<li>If you do not have any SFI to disclose, just click Next to complete your disclosure.<\/li>\n<li>If you do have SFI to disclose, when you click on Next, the SFI Entry page will display for you to fill in your information.<\/li>\n<\/ul>\n<h2 id=\"sfi-list-question\">Existing SFI &#8211; Sponsor not requiring UW review of SFI<\/h2>\n<p>If you have existing SFI and your sponsor is one that does not require UW review of SFI, you must answer the question: <strong>Does your SFI pertain to this research activity?<\/strong><\/p>\n<ul>\n<li>If you select\u00a0<strong>No<\/strong>,\u00a0then click\u00a0Next,\u00a0you will complete the disclosure. The disclosure will not include any of your existing SFI.<\/li>\n<li>\u00a0If you select\u00a0<strong>Yes<\/strong>,\u00a0an additional question displays:\u00a0<strong>Do you need to add new SFI or change existing SFI?<\/strong><\/li>\n<\/ul>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-1050\" src=\"https:\/\/www.washington.edu\/research\/wp-content\/uploads\/fids_indicate_sfi_non_phs.jpg\" alt=\"indicate S F I for non P H S sponsor\" width=\"803\" height=\"238\" \/><br \/>\nFor this question, select No or Yes.<\/p>\n<ul>\n<li>Selecting <strong>No<\/strong>\u00a0displays the Next button so you can continue.<\/li>\n<li>Selecting <strong>Yes<\/strong>\u00a0displays a list of your current SFI, if any. You can then <strong>Add new SFI<\/strong> or click on the Entity Name of an existing SFI to edit it. When finished, click Next to continue.<\/li>\n<\/ul>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-1052\" src=\"https:\/\/www.washington.edu\/research\/wp-content\/uploads\/fids_non_phs_sfi_list.jpg\" alt=\"list of existing S F I for non P H S\" width=\"810\" height=\"437\" \/><\/p>\n<h2>Existing SFI &#8211;\u00a0Sponsor requires UW review of SFI<\/h2>\n<p>A disclosure for a sponsor or originating sponsor that require UW review of SFI will automatically includes all of your existing SFI. So in this case, you just need to indicate if you want to add new or update existing SFI.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-1054\" src=\"https:\/\/www.washington.edu\/research\/wp-content\/uploads\/fids_phs_add_edit_sfi.jpg\" alt=\"question about adding or editing S F I\" width=\"808\" height=\"199\" \/><\/p>\n<ul>\n<li>If you select <strong>No<\/strong> and click Next, you will complete the disclosure.<\/li>\n<li>If you select <strong>Yes<\/strong>, then the list of your existing SFI will display, allowing you to edit any existing SFI or add new ones.
