Check the boxes that describe your work situation. Also indicate the type(s) of animals you work with currently or might in the future. The following image shows this section of the application.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-254\" src=\"https:\/\/www.washington.edu\/research\/wp-content\/uploads\/aums_app_lab_animal_use.jpg\" alt=\"lab animal use section\" width=\"609\" height=\"476\" \/><\/p>\n<h2><strong>Immunization History<\/strong><\/h2>\n<p>You must answer the tetnus and hepatitis B questions. If you select <strong>Yes<\/strong> for either answer, use the drop-down menu to choose the year. If you have missed any required fields, one or more error messages will display.
