The form consists of several sections:<\/p>\n<ul>\n<li>Human Subjects Section with responses from R&amp;R Other Project Info Form<\/li>\n<li>Human Specimens section<\/li>\n<li>If Yes to Human Subjects section, conditionally displays<\/li>\n<li>Add Studies<\/li>\n<li>Other Requested Info<\/li>\n<\/ul>\n<h2>Responses from R&amp;R Other Project Info Form<\/h2>\n<p>This section displays the answers to the Other Project Info form&#8217;s Human Subjects questions.<\/p>\n<h2 id=\"specimens\">Use of Human Specimens and\/or Data<\/h2>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-5253\" src=\"https:\/\/www.washington.edu\/research\/wp-content\/uploads\/hsct_human_specimens.jpg\" alt=\"human specimens question answered yes and showing required explanation attachment\" width=\"507\" height=\"169\" \/><\/p>\n<p>In this section, you must indicate if your research will involve human specimens or data.<\/p>\n<p>If you answer &#8220;Yes&#8221; then a link to add an attachment will display.\u00a0 Attach a PDF formatted file explaining any use of human specimens and\/or data not considered to be human subjects research. See your funding opportunity announcement and agency-specific instructions for additional guidance.<\/p>\n<h2>If Yes to Human Subjects<\/h2>\n<p>If you answered &#8220;Yes&#8221; to Are Human Subjects Involved on the Other Project Info form, this section will display.<\/p>\n<h2>Add Studies<\/h2>\n<p>Add a record for each proposed Human Subject study by selecting Add New Full Study or Add New Delayed Onset Study. There is also an option to import a study.
