You can select one or more files, and then drag and drop them or use the <strong>Browse for Files<\/strong> link. You can add attachments while your request is in Composing or Returned status.<\/p>\n<p>The following image shows the section you use to upload attachments:<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-13378\" src=\"https:\/\/www.washington.edu\/research\/wp-content\/uploads\/awards_supporting_attachments_initial.jpg\" alt=\"awards supporting attachments add files section\" width=\"625\" height=\"308\" \/><\/p>\n<h2 id=\"required_attachments\">Required attachments include:<\/h2>\n<ul>\n<li>Communication between campus and sponsors, and campus and OSP.\n<ul>\n<li>Attach emails as \u201c.msg\u201d or \u201c.eml\u201d files, this allows OSP to respond directly to email threads if needed<\/li>\n<\/ul>\n<\/li>\n<li>Award documents\n<ul>\n<li>All Award Setup Requests should include some type of Notice of Award or Agreement document<\/li>\n<li>Is there a sponsor deadline for accepting the Award? Attach sponsor documentation.\n<ul>\n<li>You will also need to Select \u201cSponsor has a deadline for accepting award\u201d in the General Information section. Explain where this date is located in the notice of award in the instructions field and attach sponsor documentation in the supporting attachments section.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><a href=\"https:\/\/www.washington.edu\/research\/wp-content\/uploads\/supporting_attachments_ASR_sponsor_deadline.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-13675\" src=\"https:\/\/www.washington.edu\/research\/wp-content\/uploads\/supporting_attachments_ASR_sponsor_deadline-1024x311.png\" alt=\"When a user selects the toggle for &quot; The sponsor has a deadline for accepting the award, additional required fields display.\" width=\"900\" height=\"273\" \/><\/a><\/p>\n<ul>\n<li>Compliance documentation, such as IACUC and IRB approvals<\/li>\n<\/ul>\n<h3 id=\"additional_requirements\">Additional Attachment Requirements for Modification Requests<\/h3>\n<p>Different types of modification requests may require a variety of other attachments. Review <a href=\"https:\/\/www.washington.edu\/research\/myresearch-lifecycle\/manage\/award-changes\/#award-modifications\">Award Changes<\/a> for more details on OSP\/GCA Modifications.<\/p>\n<h3 id=\"funding_and_budgeting_changes\">Funding &amp; Budgeting Changes<\/h3>\n<h4>Carryover- sponsor approval required<\/h4>\n<ul>\n<li style=\"font-weight: 400\">Attach a <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/concurrence-letter-sample\">concurrence letter<\/a> that details the reason for unused funds and how those funds will be spent in support of the project for OSP signature and submission to the sponsor.<\/li>\n<\/ul>\n<p><b>Addition of Equipment\u00a0<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\">Attach a <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/concurrence-letter-sample\/\">concurrence letter <\/a>and supporting sponsor required documentation, such as a copy of the quote from the equipment vendor\/manufacturer.<\/li>\n<\/ul>\n<p><b>Adding Subaward &#8211; sponsor approval required<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\">Attach a <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/concurrence-letter-sample\/\">concurrence letter<\/a> for submission to the sponsor along with any other sponsor required documentation.<\/li>\n<\/ul>\n<p><b>Supplemental Funding<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\">MODs for supplemental funding should include similar <a href=\"#required_attachments\">Required Documents<\/a> as an Award Setup Request.<\/li>\n<li style=\"font-weight: 400\">If there is a deadline for accepting the supplemental funding make sure to include sponsor documentation to that effect.<\/li>\n<\/ul>\n<p>TIP: <b>Applying for supplemental funding?<\/b> When supplemental funding requests to a sponsor require a proposal application, complete an <a href=\"https:\/\/www.washington.edu\/research\/faq\/need-use-egc1\/\">eGC1<\/a>. Note, ASRs and MODs are not used for proposals.<\/p>\n<p id=\"f_and_a_waiver\"><strong>F&amp;A Waiver Requests<\/strong><\/p>\n<p>Attach <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/fa-waiver\/\">F&amp;A Waiver<\/a> form when requesting the use of a non-standard Facilities and Administrative (F&amp;A) rate that does not fall into <a href=\"https:\/\/www.washington.edu\/research\/policies\/gim-13-facilities-and-administrative-fa-rates\/#policy\">university policy for a specific sponsored project<\/a>.<\/p>\n<h3 id=\"other_changes\">Other Changes<\/h3>\n<p><b>Federal Data Repository Access &amp; Submission<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\">Attach the completed Federal Repository form.<\/li>\n<\/ul>\n<p><b>Progress Report \/ RPPRs<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\">Not all progress reports require a Modification request. Review <a href=\"https:\/\/www.washington.edu\/research\/myresearch-lifecycle\/manage\/award-changes\/#award-modifications\">details on Progress Report submission<\/a> to determine if a modification is needed.<\/li>\n<li style=\"font-weight: 400\">For those requiring a modification: If there is not a sponsor system of submission involved, attach the progress report or other sponsor required documentation for progress report submission to the sponsor.<\/li>\n<\/ul>\n<p>If a sponsor uses an electronic system for submission of the report, <i>use that system<\/i> to prepare and forward your request to OSP (AOR). An example is eRA Commons (NIH).<\/p>\n<ul>\n<li style=\"font-weight: 400\">Attach a copy of the report with your Modification Request.<\/li>\n<\/ul>\n<p><b>Restriction Release<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\">Include supporting documentation that reflects requirement has been met (e.g. protocol approval).<\/li>\n<\/ul>\n<h3 id=\"clinical_trial\">Clinical Trial Amendments (CTA)<\/h3>\n<p>Include the following applicable attachments to your request:<\/p>\n<ul>\n<li style=\"font-weight: 400\">Amended Scope of Work<\/li>\n<li style=\"font-weight: 400\">Updated Protocol<\/li>\n<li style=\"font-weight: 400\">Current IRB approval<\/li>\n<li style=\"font-weight: 400\">Draft CTA amendment<\/li>\n<li style=\"font-weight: 400\">Sponsor\/CRO correspondence<\/li>\n<li style=\"font-weight: 400\">CTO summary review &amp; approval email with attachments \u2013 if the budget changes by 10% or there are new charges. If the CTO summary review\/approval is still pending, state this in the comments. Always provide the most current CTO summary review. Include:\n<ul>\n<li style=\"font-weight: 400\">Email response from the PI to CTO approving the CTO Summary budget review\/approval<\/li>\n<li style=\"font-weight: 400\">CTO Billing Grid and Budget attachments<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3 id=\"programmatic_changes\">Programmatic Changes<\/h3>\n<p><b>Key Personnel Changes &#8211; including PI<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\"><a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/request-a-change-of-pi\/\">Change of PI form<\/a>\n<ul>\n<li style=\"font-weight: 400\">Review <a href=\"https:\/\/www.washington.edu\/research\/myresearch-lifecycle\/manage\/award-changes\/#award-modifications\">steps for requesting a Change of PI<\/a> including <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/request-a-change-of-pi\/\">Preliminary Steps to Change a PI instructions<\/a><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\">Key Personnel changes &#8211; attach <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/concurrence-letter-sample\/\">concurrence letter<\/a> and follow <a href=\"https:\/\/www.washington.edu\/research\/myresearch-lifecycle\/manage\/award-changes\/#award-modifications\">steps to request Key Personnel Changes<\/a><\/li>\n<\/ul>\n<p><b>Work Scope Change<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\">Attach a <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/concurrence-letter-sample\/\">concurrence letter<\/a> detailing the scope change<\/li>\n<\/ul>\n<h3 id=\"schedule_changes\">Schedule Changes<\/h3>\n<p><b>Extension Requests<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\">Attach a copy of a completed <a href=\"https:\/\/forms.office.com\/r\/jKXi9gFpq2\">Extension request form <\/a>for both no-cost extension and temporary internal extension requests<\/li>\n<li style=\"font-weight: 400\">You may also need to attach a <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/concurrence-letter-sample\/\">concurrence letter<\/a><\/li>\n<\/ul>\n<p>Review guidance on <a href=\"https:\/\/www.washington.edu\/research\/myresearch-lifecycle\/manage\/award-changes\/#award-modifications\">Extension Requests<\/a><\/p>\n<h3 id=\"end_of_award_changes\">End of Award Changes<\/h3>\n<p><b>Early Termination, Relinquishments and Award Transfers<\/b><\/p>\n<p>Attach an <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/end-of-award-form\/\">End of Award Form<\/a><\/p>\n<h2 id=\"defining_attachments\">Defining the Attachment<\/h2>\n<p>Once you have uploaded an attachment, you will see several columns:<\/p>\n<ul>\n<li style=\"font-weight: 400\">File Name \u2013 the name of your file<\/li>\n<li style=\"font-weight: 400\">Type \u2013 select a value from the menu as shown below \u2013 this is a required field\n<ul>\n<li style=\"font-weight: 400\">Agreement Documents<\/li>\n<li style=\"font-weight: 400\">Cost Share Addendum<\/li>\n<li style=\"font-weight: 400\">Internal (UW) Document<\/li>\n<li style=\"font-weight: 400\">Correspondence with Sponsor<\/li>\n<li style=\"font-weight: 400\">Non Financial Report<\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400\">Description \u2013 enter a brief description of the file \u2013 this is a required field<\/li>\n<li style=\"font-weight: 400\">Attached On \u2013 displays the date and time the document was uploaded<\/li>\n<\/ul>\n<p style=\"padding-left: 40px\"><b>Note<\/b>: The maximum file size is 100MB. A message of \u201cCan\u2019t upload\u2014file exceeds 100MB limit\u201d will display if your file exceeds that size.<\/p>\n<p>The attachments will display in reverse chronological order of their most recent version.<\/p>\n<p>The following image shows a newly uploaded file, with the Type menu expanded and a red exclamation alert for the missing Description:<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-13115\" src=\"https:\/\/www.washington.edu\/research\/wp-content\/uploads\/awards_supporting_attachments_with_type_menu.jpg\" alt=\"Awards supporting attachments with one file and Type drop-down list\" width=\"946\" height=\"336\" \/><\/p>\n<p>Selecting the blue caret (&gt;) to the left of the file name will expand the row. Information about the version of the attachment will display, along with a Version Comment field.
