Faculty, librarians, and academic personnel are to complete the\u00a0<\/span><span data-contrast=\"none\">Outside Professional Work for Compensation\u00a0<\/span><span data-contrast=\"none\">(<\/span><span data-contrast=\"none\">Form<\/span><span data-contrast=\"none\">\u00a01460).\u00a0<\/span><span data-contrast=\"none\">\u00a0<\/span><\/p>\n<p>\t\t<a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/form-1460\/\"  class=\"btn btn-lg arrow purple\"><span>Download Form 1460<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/a><\/p>\n<p>\t\t<a href=\"https:\/\/apps.som.washington.edu\/OutsideWork\"  class=\"btn btn-lg arrow purple\"><span>School of Medicine Portal<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/a><\/p>\n<hr \/>\n<h2>Who needs to fill out the form?<\/h2>\n<p>All faculty, librarians, and other academic personnel, as defined in <a href=\"https:\/\/policy.uw.edu\/directory\/aps\/section-40-personnel\/aps-40-1-types-of-university-personnel\/\">Administrative Policy Statement 40.1<\/a>, who anticipate engaging in outside professional work for compensation must complete Form 1460(s). Professional or Classified staff Outside Work requests are not handled by the Office of Research. If you are a <a href=\"https:\/\/hr.uw.edu\/professional-staff-program\/\">Professional<\/a> or <a href=\"https:\/\/hr.uw.edu\/comp\/classified-staff\/overview\/\">Classified<\/a> staff wishing to engage in Outside Work, please see <a href=\"https:\/\/policy.uw.edu\/directory\/aps\/section-40-personnel\/aps-47-3-outside-consulting-activities-and-part-time-employment-by-professional-or-classified-staff-employees\/\">Administrative Policy Statement 47.3<\/a> for the correct form and process.<\/p>\n<hr \/>\n<h2>What is considered outside work?<\/h2>\n<p>Any and all professional work performed for any type of compensation organization from a non-UW organization requires approval, with limited exceptions for non-profit organizations and as noted in Additional Considerations below.<\/p>\n<p><strong>Compensation is defined broadly.<\/strong> Under EO 57, compensation includes (but is not limited to):Cash payments, salary, honoraria, and royalties<\/p>\n<ul>\n<li>Equity interests (stock, stock options, partnership interests)<\/li>\n<li>Sponsored or reimbursed travel (including lodging, meals, and registration fees)<\/li>\n<li>Intellectual property assigned, licensed, or transferred to you<\/li>\n<li>In-kind compensation \u2014 goods, services, equipment, software, hosting, or other non-cash items of value<\/li>\n<li>Compensation that is <a href=\"https:\/\/depts.washington.edu\/uwadv\/central-resources\/gproc\/non-uw-compensation\/\">waived, assigned, or directed to a third party<\/a> (including foundations, family members, or charities)<\/li>\n<\/ul>\n<p>If you are uncertain whether the anticipated work requires approval, contact <a href=\"mailto:work1460@uw.edu\">work1460@uw.edu<\/a> for a consultation.<\/p>\n<hr \/>\n<h2>When do I need approval?<\/h2>\n<p>Obtain approval <strong>before<\/strong> engaging in outside professional work. <strong>One (1) Form 1460 per organization, per year is required<\/strong> for all outside professional work. If you anticipate multiple engagements with the same outside organization within the same year, combine them into a single request, covering the full anticipated scope, days, and compensation.<\/p>\n<ul>\n<li><strong>Exception 1:<\/strong> A new and materially different relationship with the organization arises after initial approval (e.g., shifting from consulting work to a board appointment, or acquiring equity).<\/li>\n<li><strong>Exception 2:<\/strong> The originally approved scope and day count will be exceeded or the compensation will qualify as a <a href=\"https:\/\/www.washington.edu\/research\/glossary\/significant-financial-interest\/\">Significant Financial Interest<\/a> \u2014 submit a revised or supplemental Form 1460.<\/li>\n<li><strong>Exception 3:<\/strong> The engagement spans more than one year \u2014 submit one Form 1460 per year (or part thereof).<\/li>\n<\/ul>\n<hr \/>\n<h2>Why do I need to do this?<\/h2>\n<p>Requesting and receiving approval prior to engaging in outside professional work is required by UW policy, <a href=\"https:\/\/policy.uw.edu\/directory\/po\/executive-orders\/eo-57-outside-professional-work-policy\/\">Executive Order 57<\/a>. It also helps protect you from penalties for violating certain provisions of the <a href=\"https:\/\/apps.leg.wa.gov\/rcw\/default.aspx?cite=42.52\">Washington State Public Ethics Law<\/a>.<\/p>\n<hr \/>\n<h2>How do I request approval?<\/h2>\n<p>Approval is requested using <a href=\"https:\/\/www.washington.edu\/research\/forms-and-templates\/form-1460\/\">UW Form 1460 \u2013 Request for Approval of Outside Professional Work for Compensation<\/a>. Form 1460 requires signatures by your department chair and dean or chancellor.<\/p>\n<p><strong>The Form 1460 must be downloaded \u2013 do not use local downloaded versions of the Form 1460. Forms are sent and received digitally.<\/strong> Once your Form 1460 has been reviewed and signed by your department chair and dean or chancellor, send the completed form to <a href=\"mailto:work1460@uw.edu\">work1460@uw.edu<\/a>. Note that paper submissions are no longer accepted.<\/p>\n<p><a name=\"approval\"><\/a><\/p>\n<h3><strong>The Approval Process<\/strong><\/h3>\n<ul>\n<li><strong>Pre-Approval:<\/strong>\n<ul>\n<li>Download and complete\u00a0Form 1460.<\/li>\n<li>Forward the completed Form to your department chair for approval and signature.\n<ul>\n<li>Note: Some schools and colleges have additional approval requirements. Outside work meeting \u201cDeeper Involvement\u201d criteria require additional review to manage issues such as conflicts of interest. Such situations also require review by the Director of Compliance and may include Conditions of Approval that must be met.<\/li>\n<\/ul>\n<\/li>\n<li>Chair forwards Form to your dean or chancellor for approval and signature.<\/li>\n<li>Dean or chancellor (or delegated party) forwards the digitally signed Form 1460 to work1460@uw.edu.<\/li>\n<li>Contact the Office of Research at\u00a0<a href=\"mailto:work1460@uw.edu\">work1460@uw.edu<\/a>\u00a0with questions.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Processing and Determination:<\/strong>\n<ul>\n<li>The Office of Research verifies that the Form 1460 is complete.\u00a0<strong>Incomplete forms are returned to the school or college for correction before review begins.<\/strong><\/li>\n<li>The completed Form 1460 is reviewed by the designated Office of Research individual, based on the nature of the Outside Work request.<\/li>\n<li>If a Deeper Involvement with the Organization is indicated, the Director of Compliance will review the request. If further information is needed, you will be contacted to provide additional details.\u00a0<strong>Note that conditions of approval are routinely applied to requests for Outside Work with a Deeper Involvement with the Organization, and you are expected to be aware of and apply them.<\/strong><\/li>\n<li>If no conflicts are identified that would prevent approval of the Outside Work, you and your school or college will receive email notification with the approved Form 1460 attached.<\/li>\n<li>If a management plan or further information is needed, you or your school or college will be contacted.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<hr \/>\n<h2>Before you submit<\/h2>\n<p><strong>Incomplete Form 1460 submissions will be returned without review. <\/strong>Before forwarding, confirm:<\/p>\n<ul>\n<li>All form fields are filled in (do not leave blanks fields that apply to your engagement)<\/li>\n<li>The formal Outside organization name and address are provided<\/li>\n<li>Specific start and end dates are listed (not &#8220;TBD&#8221; or open-ended)<\/li>\n<li>Number of days requested is calculated and entered<\/li>\n<li>Compensation type is described (e.g., cash, equity, travel, etc.) and indicated as a Significant Financial Interest if it meets the thresholds.<\/li>\n<li>All Yes\/No questions are answered<\/li>\n<li>Department chair signature is present<\/li>\n<li>Dean or chancellor signature is present<\/li>\n<li>Same-organization engagements within the same fiscal year are combined onto one form whenever possible<\/li>\n<\/ul>\n<hr \/>\n<p><a name=\"additional\"><\/a><\/p>\n<h2>Additional Considerations<\/h2>\n<p><strong>Approval is NOT required when any of the following is true:<\/strong><\/p>\n<ul>\n<li>You are not compensated in any way (including in-kind, waived, or assigned compensation) for the outside work.<\/li>\n<li>The outside work involves contributing professional expertise related to your academic work for <strong>domestic<\/strong> scholarly, editorial, and advisory bodies, including governmental bodies.<\/li>\n<li>The work is community service on public commissions or boards of philanthropic organizations.<\/li>\n<li>Outside work is at the invitation of U.S. colleges, universities, and governmental agencies involving:\n<ul>\n<li>presenting guest lectures<\/li>\n<li>delivering scholarly papers<\/li>\n<li>serving on review panels<\/li>\n<li>participating in accreditation activities<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>The activities listed above are not subject to limitations on outside work and do not require prior approval as such. If they involve travel, however, approval for travel away from the University must be sought from the appropriate supervisor. This exclusion does not apply to teaching courses for credit or participating in continuing education activities not under the aegis of the University of Washington; prior approval for such activities must be sought.<\/p>\n<p><strong>UW Facilities, Equipment, and Resources<\/strong><\/p>\n<p>De minimis use of University facilities and resources may be permissible for approved Outside Work. For additional information regarding the University\u2019s Policy, refer to <a href=\"https:\/\/policy.uw.edu\/directory\/aps\/section-40-personnel\/aps-47-2-personal-use-of-university-facilities-computers-and-equipment-by-university-employees\/\">Administrative Policy Statement 47.2<\/a>, especially section 3.c.<\/p>\n<hr \/>\n<p><a name=\"scenarios\"><\/a><\/p>\n<h2>Scenarios<\/h2>\n<div class=\"accordion non-bold\" id=\"accessible-accordion\">\n<div class=\"screen-reader-text\">Accessible Accordion<\/div>\n<div class=\"card\">\n<div class=\"card-header\" id=\"accordion-header\">\n<h3 class=\"mb-0\"><button class=\"btn btn-link\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse\" aria-expanded=\"false\" aria-controls=\"collapse\"><span class=\"btn-text\">I have been invited to present my research at another academic institution for which I would receive a $1,000 honorarium. Does this activity require submission of Form 1460?<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/button><\/h3>\n<\/div>\n<div id=\"collapse\" class=\"collapse \" aria-labelledby=\"collapse\" data-parent=\"#accordion\" role=\"region\">\n<p>No. Invitations to make scholarly presentations, even for compensation (the honorarium), constitute one of several categories of activities that are excluded from the Outside Work Policy. Approval is not required for this presentation. (See &#8220;Additional Considerations&#8221; above for other exclusions.)<\/p>\n<\/div>\n<\/div>\n<div class=\"card\">\n<div class=\"card-header\" id=\"accordion-header\">\n<h3 class=\"mb-0\"><button class=\"btn btn-link\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse\" aria-expanded=\"false\" aria-controls=\"collapse\"><span class=\"btn-text\">I have been asked to serve on a scientific advisory board for a company for which I would be paid $15,000 per year to attend four day-long meetings. Does this activity require submission of Form 1460?<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/button><\/h3>\n<\/div>\n<div id=\"collapse\" class=\"collapse \" aria-labelledby=\"collapse\" data-parent=\"#accordion\" role=\"region\">\n<p>Yes. Providing consulting services for industry requires prior approval as outside work for compensation. Complete Form 1460 and submit it for review and approval by your department chair and school or college dean or chancellor. If the consulting period falls within a single year (Academic Year is recommended, July 1 through June 30), submit one Form 1460 covering the entire period. If the consulting period spans two or more years, submit one Form 1460 for each year or part thereof.<\/p>\n<\/div>\n<\/div>\n<div class=\"card\">\n<div class=\"card-header\" id=\"accordion-header\">\n<h3 class=\"mb-0\"><button class=\"btn btn-link\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse\" aria-expanded=\"false\" aria-controls=\"collapse\"><span class=\"btn-text\">I referee my child's sports league, for which I am paid $40 per game. Does this activity require submission of Form 1460?<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/button><\/h3>\n<\/div>\n<div id=\"collapse\" class=\"collapse \" aria-labelledby=\"collapse\" data-parent=\"#accordion\" role=\"region\">\n<p> No. Unless you are involved at UW in athletics, these activities are not considered \u201cprofessional\u201d activities with respect to your UW position. Prior approval is not required for these activities.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<hr \/>\n<p><a name=\"FAQs\"><\/a><\/p>\n<h2>FAQs<\/h2>\n<h3>Who<\/h3>\n<div class=\"accordion non-bold\" id=\"accessible-accordion\">\n<div class=\"screen-reader-text\">Accessible Accordion<\/div>\n<div class=\"card\">\n<div class=\"card-header\" id=\"accordion-header\">\n<h3 class=\"mb-0\"><button class=\"btn btn-link\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse\" aria-expanded=\"false\" aria-controls=\"collapse\"><span class=\"btn-text\">Do I need to request approval for my outside work if I have a part-time faculty appointment?<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/button><\/h3>\n<\/div>\n<div id=\"collapse\" class=\"collapse \" aria-labelledby=\"collapse\" data-parent=\"#accordion\" role=\"region\">\n<p> Yes. Time commitment is not the only factor reviewed. Form 1460 is also reviewed for conflicts with UW policies, financial conflicts of interest, and academic conflicts. <\/p>\n<\/div>\n<\/div>\n<div class=\"card\">\n<div class=\"card-header\" id=\"accordion-header\">\n<h3 class=\"mb-0\"><button class=\"btn btn-link\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse\" aria-expanded=\"false\" aria-controls=\"collapse\"><span class=\"btn-text\">Do I need to request approval for consulting that I have already disclosed in FIDS?<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/button><\/h3>\n<\/div>\n<div id=\"collapse\" class=\"collapse \" aria-labelledby=\"collapse\" data-parent=\"#accordion\" role=\"region\">\n<p> Yes. The two processes are not interchangeable. Your FIDS disclosure addresses financial conflict of interest under GIM 10. Your Form 1460 addresses approval under EO 57 and is reviewed for additional conflict types \u2014 academic, time commitment, contractual. <\/p>\n<\/div>\n<\/div>\n<div class=\"card\">\n<div class=\"card-header\" id=\"accordion-header\">\n<h3 class=\"mb-0\"><button class=\"btn btn-link\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse\" aria-expanded=\"false\" aria-controls=\"collapse\"><span class=\"btn-text\">I have a part-time administrative appointment in addition to my faculty position. If it pertains to your administrative position, obtain approval from your administrative supervisor.<\/p>\n<\/div>\n<\/div>\n<div class=\"card\">\n<div class=\"card-header\" id=\"accordion-header\">\n<h3 class=\"mb-0\"><button class=\"btn btn-link\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse\" aria-expanded=\"false\" aria-controls=\"collapse\"><span class=\"btn-text\">I am Professional or Classified staff \u2014 do I need to file a Form 1460?<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/button><\/h3>\n<\/div>\n<div id=\"collapse\" class=\"collapse \" aria-labelledby=\"collapse\" data-parent=\"#accordion\" role=\"region\">\n<p>Outside Consulting Activities and Part-Time Employment for Professional and Classified staff is governed by <a href=\"https:\/\/policy.uw.edu\/directory\/aps\/section-40-personnel\/aps-47-3-outside-consulting-activities-and-part-time-employment-by-professional-or-classified-staff-employees\/\">APS 47.3<\/a>. Staff should use <strong>Form 1555 (Outside Work and Activities Request)<\/strong> to request approval. Form 1555 is part of the University Human Resources process and is administered at the school or departmental level rather than by the Office of Research.<\/p>\n<p>Completed Form 1555 submissions are retained by the employee\u2019s school, college, or department \u2014 not by the Office of Research. UW defines an <a href=\"https:\/\/www.washington.edu\/research\/glossary\/international-nexus\/\">international nexus<\/a> to include:<\/p>\n<ul>\n<li>Travel outside the U.S. and territories<\/li>\n<li>An appointment or employment with a foreign institution, entity, or foreign government (including honorary or courtesy titles)<\/li>\n<li>Consulting or collaborating with or for a foreign person, institution, entity, or government<\/li>\n<li>Consulting or collaborating with a U.S. entity that is 50% or more owned by a foreign person, institution, entity, or government<\/li>\n<li>Receiving or providing materials, technology, information, or data to or from a foreign person, institution, entity, or government \u2014 even when the funding or compensation is from a U.S. entity<\/li>\n<li>Participation in a <a href=\"https:\/\/www.washington.edu\/research\/compliance\/foreign-interests-in-sponsored-programs\/#mftrp\">foreign talent recruitment program<\/a><\/li>\n<\/ul>\n<p>If your outside engagement falls into any of these categories, submit a Form 1460. These engagements often also trigger research security, export control, and federal sponsor disclosure obligations. Submit a supplemental or revised form if a materially different relationship emerges later, if the originally approved scope or day count will be exceeded, or if the engagement extends into a new year.<\/p>\n<\/div>\n<\/div>\n<div class=\"card\">\n<div class=\"card-header\" id=\"accordion-header\">\n<h3 class=\"mb-0\"><button class=\"btn btn-link\" type=\"button\" data-toggle=\"collapse\" data-target=\"#collapse\" aria-expanded=\"false\" aria-controls=\"collapse\"><span class=\"btn-text\">What is the maximum number of outside work days I\u2019m allowed?<\/span><span class=\"arrow-box\"><span class=\"arrow\"><\/span><\/span><\/button><\/h3>\n<\/div>\n<div id=\"collapse\" class=\"collapse \" aria-labelledby=\"collapse\" data-parent=\"#accordion\" role=\"region\">\n<p>Generally, faculty are allowed to work up to one day (8 hours) per week with an outside organization. A 100% FTE 12-month appointment is eligible for up to 52 days per year. A 100% FTE 9-month appointment is eligible for 39 days during the nine UW months and 65 days during summer (104 days total). Part-time appointments are prorated.
