For the Record- May 3, 2022: Updated Exempt Guidance, Zipline Account Creation

Updated Guidance and New Consent Template for Exempt Research

Implemented 4/28/2022

New template. As part of HSD’s ongoing Consent Revision Project, we have published a new consent template: TEMPLATE Consent, Exempt Research. It is important to know that:

  • HSD does not review or approve consent materials for exempt research and consent materials should not be uploaded to Zipline for these studies
  • Researchers are responsible for providing subjects with particular information prior to their agreement to participate in the study

This optional and highly editable template is intended to assist researchers with providing this information. HSD has also posted examples of what a consent form for exemptions might look like depending on the study design ( EXAMPLE Consent, Exempt Research).

Updated guidance. As part of another ongoing project, HSD is turning Word-based guidance documents into webpages and consolidating them into a single Guidance section on our website. With the creation of the new template, we took the opportunity to transform the GUIDANCE Exempt Research from a Word document into a webpage. As part of that revision, information about what would exclude a study from being exempt and the details about each exempt category were relocated from the retiring WORKSHEET Exempt Determinations into the web guidance. This means there is now a single location for all information about exempt research.

Zipline Account Creation

To be implemented 5/31/2022

The evening of May 31st, Zipline will be down between approximately 6 pm and 7 pm for maintenance and minor upgrades, including the re-implementation of Zipline self-registration for most new users.

Once access to Zipline is restored, most new users will be able to create their own accounts by logging in with their UW NetID and password. New users with a sponsored UW NetID may still need to contact hsdinfo@uw.edu to request a Zipline account.

The instructions for new account creation will be updated on May 31st, 2022, when the Zipline changes have been made.

Return to In-person PAVE Visits

Update

The Post-approval Verification and Education (PAVE) program is now offering researchers the option to have their PAVE visit conducted in-person or remotely. Your preference will be solicited during the scheduling process. For in-person visits, we will abide by any COVID-related restrictions your location requires (e.g., masking in clinical care areas of UW Medicine).

Slower Turnaround Times in July and August

Update

The past 2+ years have been a difficult time for all of us. With COVID-19 and travel-related restrictions now lifting, we anticipate many HSD staff will be taking time off this summer to recharge and recuperate. While we will ensure that there is adequate coverage, we anticipate that turnaround times will be slower in July and August. We appreciate your patience and understanding. To minimize the impact to your research, we encourage you to ensure your applications are complete and submitted as early as possible. For any externally imposed urgent deadlines, please be sure to call these out in your submission so we can prioritize accordingly.
