Long-Term Disability (LTD) Optional Insurance
Faculty, Staff & Librarians — Monthly Premiums

Optional LTD plan benefits include a retirement supplement feature, so the premium rate you pay is determined by both the waiting period you select and the retirement plan, if any, in which you participate.

Optional LTD Monthly Premium Rates

Effective 1/1/2012 - 12/31/2014

To calculate your Optional Plan LTD premium, multiply your monthly base pay (up to $10,000) by the percentage shown for the desired Benefit Waiting Period below.

Examples:

A UW Retirement Plan employee who earns $3,000/month and is enrolled in the 60-day benefit waiting period would pay $28.50 per month (0.95% x $3,000 = $28.50).

Other employees (those in TRS, PERS, or LEOFF Plans or not yet enrolled in a retirement plan) who earn $3,000/month and enrolled in the 60-day benefit waiting period would pay $23.40 per month (0.78% x $3,000 = $23.40).
