Radio Buttons

Radio buttons are used to force a single selection out of two or more choices that are shown in advance. Note: generally, radio buttons are not used if there are more than 5 choices. Instead, selection lists are used to reduce clutter.

Checkboxes

Checkboxes are used to allow multiple selections out of a group of answers that are shown in advance. Note: Similar to radio buttons, checkboxes are not generally used if there are a large number of choices.

Selection List

Selection lists appear as pull-down lists. They are used to allow selection of one choice from a set of choices that are not shown in advance.

Textbox

Textboxes are used to allow a single line of user input. If multiple lines of input or an unknown number of lines are needed, a textarea field should be used.

Textareas are boxes used to allow multiple lines of user input.
