Academic Adviser Performance Evaluation

This evaluation instrument was prepared in 1995 by a committee of the UW Association of Professional Advisers and Counselors. Advising offices are welcome to print it and use it, or modify it to better serve their needs.

• Performance Evaluation Form--the short form. This may be all you need.
• Recommended Procedure--an overview of the recommended evaluation procedure, followed by a detailed explanation.
• Academic Adviser Responsibilities--expands on the areas of responsibility to be evaluated, with examples of each performance level ("exceptional," "exceeds expectations," etc.)
• Student Evaluation Form--created by Undergraduate Advising; departments will want to modify it, or skip it altogether
