Petitions

Petitions to waive tuition forfeitures and u-pass charges may be submitted to Student Fiscal Services either by mail, fax or in-person. All petitions will be considered on a case-by-case basis. Your petition will be processed by the Student Fiscal Services Office once all information is complete and required documentation has been received. The petition, with complete documentation, must be submitted by the last day of the quarter in which the charge is assessed.

Tuition Forfeitures

Tuition forfeiture fees have specific guidelines or reasons where fees may be waived. Read the guidelines carefully before submitting your petition. You may download a tuition forfeiture petition form and, if necessary for documentation, a Health Care Provider Verification Form, or pick the forms up in person at Schmitz Hall, room 129. This form is used on the Seattle campus only. The petition must be submitted by the last day of the quarter in which the charge is assessed. If you are a student at Bothell or Tacoma, please contact the Student Affairs Office on your campus for the proper forfeiture petition procedure.

U-Pass Charges

If you have lost, or have not received your U-Pass, you may file a petition with the Student Fiscal Services Office. To submit a petition for the removal of the U-Pass fee, print out the U-Pass petition form, fill it out and return it to Student Fiscal Services in Schmitz Hall, Room 129. Petition forms are also available in the Student Fiscal Services Office.


Documentation is required for all tuition forfeiture petitions. Documentation may be faxed, mailed or emailed to Student Fiscal Services. Written documentation must be on official letterhead and signed. Documentation that is emailed must include the full name, title, address or department and phone number of the person verifying the facts of the petition in addition to your full name and student number. Your petition cannot be processed without complete documentation.
