Forms for Proposals
Use of Checklist
Several sponsors, e.g., Public Health Service, American Cancer Society, National Science Foundation, provide application kits (forms and instructions) for proposals.
Others, e.g., Air Force and NASA simply provide a guide to be followed in proposal preparation. Some Federal sponsors such as the Departments of Education and National Oceanic and Atmospheric Administration (NOAA) use the standard SF-424 Federal Grant-in-Aid Form. These forms and guides may be requested directly from the sponsor, or may be available locally from Office of Sponsored Programs. In those instances where neither an application kit nor a guide is provided, Office of Sponsored Programs can assist in shaping the proposal.
Appendix I provides a checklist of items that should generally be considered in the preparation of proposals.
Appendix II provides an example of a cover page for those instances where the sponsor does not specify some other format.
Appendices III, IV, and V, provide examples of budget presentations. Certain budget items are discussed in Section III.
