UWise Information (UW IRB Safety Extranet)

In order to better serve the needs of our users, deployment of UWise has been halted. Please see the instructions below for additional information on how applications already submitted via UWise should be handled.

March 10, 2006
No new applications, modifications, or status reports may be created in UWise, after this date. For applications or materials that have already been started or submitted through UWise, please see the information below to determine how to proceed.

April 14, 2006
Last day to accept through UWise any applications, modifications, or status reports that were created in UWise prior to March 10.

July 1, 2006
Studies that will close by this date may stay in UWise until the study is closed, but new modifications may not be created after April 14.

For applications, modifications, or status reports that have already been started or submitted through UWise, please see the information below to determine how to proceed. If STATUS = Researcher Prep
(application started but not yet submitted to HSD)
There are three options. You may choose the one that best suits your needs.

  1. Complete the application in UWise and submit it to HSD via UWise by April 14.
  2. Withdraw the UWise application and prepare a paper-based application instead.
  3. Complete the application in UWise by July 1, then print it, and send one copy to HSD as a paper-based application. Your application should include a completed and printed copy of Page 1 of the standard paper Human Subjects application, in order to satisfy the requirement for departmental approval.

If STATUS = Committee Pre-review or Review
(application submitted and under review)

  • All activities will continue to occur in UWise, until the application has been approved.
  • After approval, the Human Subjects Division will print the entire application and all electronic documents associated with it, and then create a paper file for the study. We will send you a copy.
  • The application and other materials will remain available to you on a “read-only” basis until shortly after you have received the printed copy. During this time, you will be able to print copies yourself, or create electronic copies (PDF files). However, you will not be able to edit or revise any materials.

If STATUS = Researcher Revision
(new application submitted & reviewed; researcher preparing response to review)

  • All activities will continue to occur in UWise, until the application has been approved.
  • After approval, HSD will print the entire application and all electronic documents associated with it, and then create a paper file for the study. We will send you a copy.
  • The application and other materials will remain available to you on a “read-only” basis until shortly after you have received the printed copy. During this time you will be able to print copies yourself, or create electronic copies (PDF files). However, you will not be able to edit or revise any materials.

If STATUS = Approved

  • Any activities already begun in UWise as of March 10, 2006 and submitted via UWise by April 14, 2006 (such as a Modification Request or Status Report) will be completed and approved in UWise. After April 14, Modification Requests or Status Reports will be completed and submitted on paper.
    • HSD will print a complete copy of the entire application and all documents associated with it, and then create a paper file for the study. We will send you a copy.
    • The application and other materials will remain available to you on a “read-only” basis until shortly after you have received the printed copy. During this time you will be able to print copies yourself, or create electronic copies (PDF files). However, you will not be able to edit or revise any materials.
