Compliance Explanations : Animal Use

General Information

Compliance questions are asked to:

Identify special considerations in this application and bring them to the attention of reviewers

Provide information necessary for the UW approval process

Provide information that may help campus unit and central office approvers to review your application quickly

Each yes/no question must be answered, and each question answered "yes" will require additional information or explanation. Some answers will also cause an approver or watcher to be added to the Signature Page or Approval Graph.

This table lists all the fields on the Animal Care section of the Compliance Explanations screen of the eGC1.

AC-1-A	For each facility, enter (or copy and paste) the following information into the text box: Name of Facility Indicate the facility where the work will take place (e.g., "UW main campus", "field location", "San Diego Zoo"). Facility Location Indicate the location where the work will take place (e.g., "Seattle, WA", "Marrowstone Island, WA", "San Diego, CA"). Animal Welfare Assurance Number Indicate the Animal Welfare Assurance Number for each facility listed above (e.g., "A3187-01" for the University of Pittsburg). IACUC Protocol Number List all relevant IACUC protocol numbers. IACUC Approval Date Provide the most recent IACUC approval date for each protocol number listed above. Provide a letter from the IACUC office of each of the facilites cited. The letter must confirm the protocol number and approval dates listed above, and it must indicate that the animal work is approved as described in the grant. A confirmation letter is not required from institutions that have inter-institutional agreements with the UW, such as the Fred Hutchinson Cancer Research Center. However, for those institutions, the protocol(s) cited must be filed with the UW IACUC office. If they are not already on file, please provide them.	A list of domestic Assurance Numbers

Animal Welfare Assurance Number Indicate the Animal Welfare Assurance Number for each facility listed above (e.g., "A5195-01" for the University of London). Also provide: A letter from each foreign institution confirming that they have approved the protocol, and the date of approval.
