Documentation of Training
Trainees will not receive "certificates
of attendance" that
verify training. Instead,
training can be verified
by choosing one of the following
options:
Important
note: The National
Institutes of Health require
signed verification, as in
option #1.

PIs can use this template
to compose a letter to the
funding agency verifying
that all key grant personnel
have completed the required
training in the responsible
conduct of research. In order
to complete this letter,
PIs will need to go to the
HSD website and refer to
the list of people who have
completed the required training.
PIs should complete, sign,
and submit this letter to
Office of Sponsored Programs
(OSP) along with the grant
proposal. The letter will
be verified and cosigned
by OSP.
In situations where
OSP co-signature is required
but group verification
letters are inappropriate,
this template can be edited
in order to compose an individual
verification letter. In such
cases, the individual trainee
should sign and submit the
letter to OSP, as described
above.

Individuals
can use this template to
compose a letter verifying
that they have completed
the required training in
the responsible conduct
of research.
