Any time research changes from what was approved by the IRB, a Modification form, requesting approval of the change, must be submitted to the Human Subjects Division. The Modification form must be submitted and approval received before the change is implemented.

Modifications receive full IRB review when the change is substantive and may alter the risk and/or benefit to subjects who participate in the research. When a change is minor in nature and will not alter the risk and/or benefit to subjects, the Modification may undergo review via an "expedited" process. Rather than full IRB review, the Modification may be reviewed by a sub-group of the IRB.
