Report a Problem With My Research / Study

How to report a problem:

Only report problems that you believe are related, probably related, or possibly related to your research activities or procedures.

For any event or incident that occurs during your research study, if it is not considered a medical, clinical, or “physical” (meaning something that happened to a human body) event, or a psychological harm, use the Report of Other Problems form (see "Related Documents" on this page). Generally, this form is used to report protocol violations, deviations, and other unanticipated problems or events.

How to report an Adverse Event:

Only report problems that you believe are related, probably related, or possibly related to your research activities or procedures.

Adverse Events are medical, clinical, or “physical” (meaning something that happened to a human body) in nature, or a psychological harm. If the researcher thinks the Adverse Event is serious, please call HSD at 206-543-0098 to report it within 1 business day, and also complete an Adverse Event form (see "Related Documents" on this page).
