Purpose
The purpose of this form is to provide the University of Washington (UW) Human Subjects Division (HSD) with the information that is necessary to certify submission of data to the National Institutes of Health (NIH) Genome-Wide Association Studies (GWAS) database of Genotypes and Phenotypes (dbGaP).
