This form must be completed and reviewed by the IRB for all UW human subjects research involving any component of the U.S. Department of Defense (DOD), except research that qualifies for "exempt" status.

This document is for use with paper-based applications only. Do not use with Zipline. HSD accepts versions from 3/27/2015 on.
