Authorization to Apply for a Banquet Permit Form Instructions

A University of Washington "Authorization to Apply for a Banquet Permit to Serve Alcoholic Beverages" form is required for all individuals and/or organizations that desire to use UW facilities for service and consumption of alcoholic beverages at a banquet or organized gathering at a specified date and place. For more information, please consult UW Administrative Policy Statement 13.9.

The Banquet Permit authorizes only the serving and consumption of alcoholic beverages. If you plan to sell alcoholic beverages at your event and/or if the event is to be sponsored financially or with gifts/donations of alcoholic beverages by representatives of the alcoholic beverage industry (marketers, distributors, or producers of alcoholic beverages), then additional requirements apply .

Instructions for Completing the Authorization to Apply for a Banquet Permit to Serve Alcoholic Beverages form:

The instruction provided below match the numbers on the electronic form.

1. Applicant Name:

Name of the person making the request. This person accepts responsibility for compliance with Washington State law, Washington State Liquor Control Board regulations, and UW policies and procedures. This person has read, understood, and agreed to the terms of the Banquet Permit to Serve Alcoholic Beverages Agreement.

2. Status:

Select one (Faculty, Student, Staff, Registered Student Organization, or Other).

4. Applicant Address / Box Number:

Applicant’s address and/or UW Box Number.

5. Applicant Phone:

Applicant’s phone number.

6. Applicant Email:

Applicant’s email address. The applicant will be updated on the form’s status through this email account.

7. Applicant's Department/Organization:

Applicant’s department or organization.

7a. Approving Dean/Director/Chair/SAO name (student applicants only):

Provide the Dean, Director, Chair, or SAO Representative’s name.

Sections 7a and 7b only apply to student applicants. If a student is organizing a party or is organizing an event for an RSO, the approval of a Dean, Director, Chair, or SAO representative (Rene Singleton) is required. This is in addition to the logistics approval from the Venue Operator and the UUFC final approval. RSO events are always approved by Rene Singleton. Other events will need the support of a Dean, Director, or Chair.

7b. Approving Dean/Director/Chair/SAO email (student applicants only):

The Dean, Director, Chair, or SAO representative’s email address. This person will be asked for approval through email. RSO events should use sniglet @ u.washington.edu.

8a. Event Name:

Event name.

8b. Description of Event and Purpose for Serving Alcoholic Beverages:

Describe the event and the purpose for serving alcoholic beverages.

8c. Is this event being supported by a wine, beer, or liquor manufacturer or distributor?:

Is the event receiving a donation of alcohol or any other kind of financial assistance from an alcohol manufacturer or distributor? Will this event promote or advertise an alcohol manufacturer or distributor’s products? If yes, please indicate this and provide the name of the manufacturer or distributor.

9. Estimated Attendance (numerals only):

Provide your estimated attendance. If the under-21 ratio of attendees is high, the form may be subject to greater scrutiny.

10. Event Location:

Select the location of your event from the drop-down. If the event is in an unusual venue or is not listed in the drop-down—such as the parking lot between Balmer and Mackenzie Hall or the Dental School student lounge—for example, enter that information in "Other Location." If the event is in "Other Location," provide the name and email of your venue operator or room reservationist. In special situations like this you may already have this information, or, you may contact us for assistance.

11. Event Date and Time:

Enter the event’s date and time.

12. Compliance Responsibility:

Accept responsibility for compliance.

Reservation and Approval Procedures

Prior to completing the electronic form, the individual making the request should: (1) make a tentative reservation with the venue, and (2) if the applicant is a student or Registered Student Organization, gain the approval of a departmental or Student Activities Office sponsor. This person should be a Dean, Director, Chair, or Assistant Director of Student Activities Rene Singleton for an RSO event.

The electronic form should then be filled out as completely as possible prior to submission.

To gain approval to serve and consume alcohol on campus:

• Complete the electronic Authorization to Apply for a Banquet Permit to Serve Alcoholic Beverages form and submit it at least two weeks before the event date.

• If a student or RSO is the applicant, the form is first forwarded to the Dean, Director, Chair, or SAO representative for approval. (If the applicant is not a student or RSO, the form skips this step).

• After Dean/Dir/Chair/SAO approval, the electronic form is forwarded to the venue for logistical approval or disapproval.

• At the end of the process, the form is routed to the Chair of the Use of University Facilities Committee for final approval or disapproval.
