Apply to the MEDEX Extended Masters in Clinical Health Services

The EMCHS is currently accepting applications for the first cohort of students for the 2010 - 2011 program, which starts summer quarter 2010. The deadline for all application materials to be received is May 29, 2010, 5:00 p.m. Pacific Standard Time. We consider the date on which applicants have submitted all application materials when we determine whether the application deadline has been met.

UW Graduate School Application

To begin the application process, go to the UW Graduate School Online Application. The Graduate School requires students to submit an application separate from that submitted to the academic department. You must submit the Graduate School Application electronically. More information about The EMCHS Degree offered through UW Education Outreach can be found at http://outreach.washington.edu/emchs/

  1. When submitting the online Graduate School application form, you will be asked to create an account.
  2. Once you have created an account, you will be asked to select the graduate program to which you are applying. Please select "Extended Master of Clinical Health Services” as your choice in the pull-down menu. As part of the Graduate School Application Process you will be required to submit basic demographic information including prior education, a personal statement, and a copy of your curriculum vitæ.
  3. After completing the essential components of the Graduate School application, be sure to pay your fee and to print a copy of your Graduate School Application—you will sign this and include it in the package of materials you mail to UW Educational Outreach, who will work with MEDEX Northwest to process your application. The UW Graduate School Application Fee is $65 and is due at the time you officially submit your application.

EMCHS Required Supplemental Application Materials

Completing the Graduate School online application allows you to pay your fee and print out an official Graduate School Application form that you must sign and send in. In addition to this form, you must also ensure that the following supplemental materials are mailed to UW Educational Outreach:

  • One official copy of your transcript from the college or university where you earned your highest degree.
  • An official letter of support from your current employer specifying that time will be allowed for participation in the program. A letter of support is not required for applicants not currently employed. If an applicant becomes employed prior to enrollment or during the program, an employer’s support letter must be obtained and submitted to MEDEX Northwest.
  • Documentation of NCCPA certification
  • TOEFL Scores (If Applicable), See UW Graduate School English Language Proficiency Requirement Policy for more details.

Important note: All of the above application materials must be received before the admissions committee will review your application.
