Getting Ready for your Mary Bridge Peds Rotation

The Pediatric WWAMI Medical Student Program will contact you via email approximately 5-6 weeks prior to the start of your rotation. This email will request that you provide the following information:



Group Health

(This item does not need to be completed by WRITE students)

For your six week Mary Bridge Pediatric Clerkship, students will have a few days at Group Health for their outpatient experience. You are required to complete the following forms:

  1. MHS intake form – Please complete the upper half of the first page, all above "System Access", beginning with marking the box "Medical Student", followed by completing the next two boxes. To assist you in the following sections: School or Residency - school information is as follows: "UWSOM", 1959 NE Pacific Street, Seattle, WA 98195. School Coordinator: Carla Salldin Carla.salldin@seattlechildrens.org 206-987-2063 (WRITE students only, Mylinh Nguyen, (Mylinh.nguyen@seattlechildrens.org 206-987-75869) and fax number for both 206-985-3157.
    Location section, mark "Mary Bridge".
  2. Housing Responsibility Form – this form is required to get your housing access
    MHS HIPAA/Needle stick
  3. Drug screen is required: a 6 panel oral drug screen must be completed prior to the start of the rotation. The 6 panel oral fluid drug screen can be completed through the UW SOM. For more information visit the SOM web page on drug testing for select clerkship rotation. Please contact Chakrya Lim, Program Operations Specialist in the Office of Academic Affairs limc@uw.edu, regarding verification of the completed drug screen. You will not be able to start at Mary Bridge Health Services if your completed drug screen has not been verified with Mary Bridge prior to your arrival.
  4. Flu vaccine required (including verification) during flu season.
  5. Send a jpeg picture of yourself for your MHS ID student badge. (Head shot, plain color background image)
  6. MHS HIPAA/Needle stick

You will not be able to start at Mary Bridge Health Services if you have not completed the screen and the have been communicated prior to your arrival.


Prior to your arrival

On the day of arrival, you will be given a key pad code and shown where the apartment is located. If you require check in prior to the first day of your rotation, you will need to make arrangements at least one week in advance by calling Kareena Andreas at (253) 403-8684. Prior arrival may not be possible in all cases, please check prior to coming early.
