Getting Ready for your Mary Bridge Peds Rotation

The Pediatric WWAMI Medical Student Program will contact you via email approximately 5-6 weeks prior to the start of your rotation. This email will request that you provide the following information:

Student Biographical Information (Word doc)


The following two forms (Information Services Request Form and Housing Responsibility Form) need to be returned to Kareena Andreas at least 3 weeks prior to the start of the rotation

Information Services Request form – Please complete only the following items:

  • Name to include middle initial
  • Date of birth as in MM/DD
  • Job title:  medical student
  • Date of access
  • End date
  • Signature page one
  • Signature page two and with a clear printing of your name - I.S. will need to read it accurately

Housing Responsibility Form – this form is required to get your housing access

Prior to your arrival

On the day of arrival, you will be given a key pad code and shown where the apartment is located. If you require check in prior to the first day of your rotation, you will need to make arrangements at least one week in advance by calling Kareena Andreas at (253) 403-8684. Prior arrival may not be possible in all cases, please check prior to coming early.
