The FACMAN Project Input Form is provided to list and monitor the renewal and improvement requirements for buildings, utilities, facilities, and all the infrastructure of the University of Washington Seattle campus.

Due to multiple levels of information which are touched by various groups on the campus, the project input form is prepared to simplify the input format into the FacMan database.

Facilities Services Engineering Staff and Managers, Zone Maintenance Managers and Maintenance Coordinators are the primary users of the FACMAN form.

The New FACMAN Input Form

By pressing the Tab key or clicking with your Mouse into the fields, enter information about who you are and what project information you have.

  • Red fields have ‘pull-down’ selections. A pull down arrow will appear when box is selected.
  • Black fields are filled in automatically when you select a ‘building name’.
  • Blue fields are ‘fill-in the blank’.

    1. Enter Information About Yourself as a Contact
    • Contact Name. (required) Your name.
    • Phone #. (required) Your office telephone number.
    • Email Address. (required) Your email address.
    • Office Location.(optional) Your building and room number.
    1. Enter Information About Your FACMAN Project Input Request:
    • Bldg Name. (required) Select the Building Name.
      1. Press the down arrow button.
      2. Choose from the list.
    • Fac Num. Facility Building Number. Building number identifier. Fills in automatically from the selected Building Name.
    • Address. Facility Building Address. Fills in automatically from the selected Building Name.
    • Facility Code. Facility Building Code. Building code identifier. Fills in automatically from the selected Building Name.
    • Long Name. Full Facility or Building Name. Fills in automatically from the selected Building Name.
    • Site. Facility or Building Site location. Fills in automatically from the selected Building Name.
    • Project Title. (required) Enter Project Name.
    • Project Location. (required) Enter Project Location.
    • Project Type. (required) Select the appropriate project type if known.
    • Maintenance Zone. (optional) Select appropriate maintenance zone if known.
    • Area. (optional) Select appropriate organization if known.
    • Class. (optional) Select appropriate project management group if known.
    • Impacts. (optional) Select appropriate impact to building, facility, or utility.
    • Priority Rating. (optional) Select appropriate rating.
    • Description. (required) Enter a full and complete description of the requested project.
    • Justification. (required) Enter a description of the justification for this work, including any previous works orders. Include Department contacts, locations, telephone numbers, and email addresses.
    • Facilities Services Director. (optional) Select appropriate maintenance or operations Facilities Services Director if known.
    • Assigned Engineering Services Engineer. (optional) Select assigned architect or engineer if known.
    • Project Number. (optional) Enter project number if available.
    • Budget Number. (optional) Enter project budget number if available.
    • Project Funding Amount. (optional) Enter project funding amount if available.
    • Estimate Amount. (optional) Enter project cost estimate amount if known.
    • Estimate Date. (optional) Enter date project cost estimate amount was developed if known.
    • Estimate Type. (optional) Select project cost estimate type if known.
    • Energy Item. (optional) Reply if project is related to energy conservation.
    • Design/Construction Deficiency. (optional) Reply if project is related to a design or construction deficiency.
    • Reason. (optional) Select appropriate reason.
    • Building Element Involved. (optional) Uni-format Information. Select the building element that best describes the project work.
    • Element Number. The associated assigned identifying uni-format element number. Fills in automatically from the selected Building Element Involved.
    • Engineering Services Group. The assigned planning and budget group within the project planning request. Fills in automatically from the selected Building Name.
    1. Submit the FACMAN Input Project Request. After all information is entered and the form is completed send a copy of this form by email as an attachment to Facilities Engineering, Robbie Avila (Manager for Architectural, Environmental, and Electrical) telephone number 206-221-4391, or Ali Ferdos (Manager for Mechanical, Civil, and Structural) telephone number 206-685-1408. Information is also shown at the bottom of the form.
    • A FACMAN Number will be generated when the Input Form is formally input in to the FACMAN system. This is a preliminary number. Once the project request has been APPROVED by CASPO, a Project Number will be assigned.
    • Enter another FACMAN Project Input Request if necessary.

Printing the FACMAN Input Request

Copies can be printed at any time using the right mouse button and printed via a local printer.

Editing a FACMAN Input Request

FACMAN Input Requests can only be edited or deleted while they are with the requestor. Once they’ve been forwarded and ACCEPTED by Facilities Engineering you can change or cancel a input request by calling the appropriate Facilities Engineering Manager. To edit a request follow these steps.

  1. Open the specified FACMAN Input Request form.
  2. Follow the steps as required to fill-in a new form.

Searching FACMAN for Input Requests

To search for FACMAN project inputs and requests begin by opening the FACMAN database program.

  1. Using the BMAR and FACIMP browse selection, searching for FACMAN projects can be done. Utilizing filters can speed your search.

Viewing information about the FACMAN Project Request

After the FACMAN project request has been accepted by Facilities Services, information from the FACMAN input form will be directly input into the FACMAN database and can be viewed.

  • FACMAN Number. The number assigned after request is ACCEPTED by Facilities Services.
  • FACMAN Contact. Who to contact within Facilities Services about the FACMAN database and request.

Email FACMAN Input Request Form

Utilizing the input request forms, an email with the attached form can be sent to the selected Facilities Engineering Services Manager. This form does not support viewing or tracking of your FACMAN input request, but is solely used for project input into the FACMAN database.
