Capital Projects Fluorescent Lamp Service Request Form

All fluorescent lamps removed during a Capital Project must be recycled appropriately. UW Recycling coordinates this service with our vendor Ecolights. Note: signatures are required for delivery and pickup.

Delivery of Drums for Collection of Lamps

Fill out this form at minimum 1-2 weeks prior to the removal of lamps. UW Recycling schedules the delivery of drums with the vendor. Please allow 5-10 business days for delivery.

Pickup of Full Drums

Fill out this form to schedule pickup of full drums. You can request service as drums are filled or when the project is complete. UW Recycling schedules the pickup of drums with the vendor. Please allow 5-10 business days for pickup.
