Online Mental Health Screening

Take the opportunity to fill out a brief self-assessment test for Depression, Bipolar Disorder, Generalized Anxiety Disorder, Alcohol or Post-Traumatic Stress Disorder.  Since people can suffer from these disorders at the same time, or one can cause symptoms that seem like the other, you may want to take all of the screenings offered. Each screening will take 4 to 5 minutes to complete. Your screening score is informational, not diagnostic.  It indicates whether your responses are consistent with Depression, Bipolar Disorder, Generalized Anxiety Disorder, Alcohol or Post-Traumatic Stress Disorder and where to call for help.  It is not intended to be used as a substitute for a visit with a health care professional. The University has counselors available to meet with you and discuss the results of your online screening at your request.

When you click on the link below, you will leave the University of Washington’s web page.  The University does not manage or control web sites not owned by the University.   A core value and objective of this screening is to preserve participants’ anonymity.  The University cannot identify you through your responses.  The only information that can be reported to the University is aggregate in nature and pertains to general information regarding the number of screenings that take place, the results of the screenings, and general demographic information.  If you prefer to not provide that information, each demographic question allows a “no response” answer.  Answering “no response” will have no impact on the results of the test.
