Various parties request that the University fill out these forms. When entities pay the University income (typically for sales and service activities), they are required to submit an information return to the IRS. In order to do so, you need the University's correct taxpayer identification. This form is already filled out. As UW employees are requested to provide this information, they can print out this form and sign.
