Claims Services

The Office of Risk Management manages all claims and lawsuits which allege that negligence resulted in injury or property damage to third parties. Claims include: professional liability, general liability, auto liability and employment practices liability.

How to file a claim:

If you would like to file a claim against the University of Washington, please complete the following form(s):

Or to request a claim form by mail, send e-mail to claims@u.washington.edu with your name, mailing address and for what type of incident you are requesting a claim form.

Once completed, send these forms, along with any related documentation, to:

If you’ve received a notice of a claim of lawsuit involving your duties as an employee, student or agent at UW:

If you have been named in a lawsuit and have received a letter advising you on how to request defense and indemnification from the University, here is the suggested sample letter for you to use to make your request:

If you are a Department Head whose employee or resident has been named in a lawsuit and have received a letter advising you to affirm that this person was in the course and scope of their UW duties, here is the suggested sample letter for you to use to make your request

If you've been in a vehicle collision:

If you’ve been involved in a vehicle accident while driving a UW vehicle, follow these directions.
If the third party involved in the accident contacts you regarding the accident, please refer them to our office at (206) 543-3657.
