Proof of Insurance CoverageRequest a Certificate OnlineProfessional Liability (Malpractice) CoverageThe University does not normally issue individual certificates of liability insurance coverage to each of its 23,000 employees and 35,000 students. The following information is meant to answer questions you may have about the University’s malpractice coverage. Carrier: University of Washington (the University is self-insured) Policy Number: Not applicable; this is a statutorily self-insured program Limits: Coverage is unlimited per occurrence and in the aggregate Deductible: None Term: Coverage begins on the date of appointment and continues until the appointment ends. Form: Coverage operates on an occurrence basis; “tail” coverage is not needed. Defense will be provided for claims arising from the period of appointment. Coverage: Coverage applies to the negligent acts or omissions of the University of Washington and its employees, students, and agents acting in the course and scope of their University duties. The term “agent” includes volunteers to authorized University programs. General and Automobile LiabilityThe Office of Risk Management also issues certificates providing evidence of coverage for events, vehicles, and UW employees. For events, please provide the name of the event, the dates and place where it will be held and the name of the sponsoring UW department. Please also provide the name, address (or fax number) of the person who should receive the certificate. Request an evidence of coverage certificate by calling (206) 543-3659. LinksVisiting Medical Students, Residents and Fellows have specific insurance requirements that must be fulfilled prior to their visiting the University of Washington. In the past, UW provided a master group program that provided professional liability coverage for visiting medical students, residents and fellows. However, this insurance policy was cancelled effective December 31, 2003.
