You will need to complete two forms to print your course pack:

1. Copyright Permissions Form

The Copyright Permissions Form is necessary if you are reproducing copywritten materials in your course pack. The Creative Communications Copyright Permissions Center handles all of the details involved in getting permission to reproduce these materials.

Lastly, we will need some specific information for production of your course pack. This information is included on the Course Pack Request form.
