Direct Deposit

If you complete the Direct Deposit Authorization
form, your payment will
be electronically deposited into your bank account each
payday.
You will receive an Advice
of Deposit/Earnings Statement each payday rather than a regular check.
The information provided on the Advice of Deposit is identical to the
information on a paycheck.
The Direct Deposit form is also used if you want to change the bank
account where your payment is deposited or to cancel direct deposit.
Please note that the Payroll Office must receive the signed Direct
Deposit Authorization Form at least 7 days prior to payday to start,
change or stop direct deposits. The direct deposit tape is created
the night of payroll processing and cannot be altered.
It saves you time:
Your net pay is automatically deposited to your checking or savings
account, even when:
- You are on vacation
- You are traveling
- You are sick
Direct deposit is safe:
- No worry about lost or stolen paychecks
- No worry about forged paychecks
- No danger of fraud
- If you want to authorize direct deposit, you may
obtain a form from this
website, your department, or by calling the Payroll Office forms line
at
543-9203.
- Complete the form, attach either a voided check or deposit slip which
reflects your bank account number.
- Mail the form to the Payroll Office at 3903 Brooklyn Ave. NE, Box
355655, Seattle, WA 98105-6694.
- When signing up for direct deposit, please allow two pay periods
for the direct deposit to take effect.
- To change your direct deposit, complete a new Direct Deposit Authorization form.
- Attach a deposit slip or voided check which reflects your new bank account
number.
- Mail the form to the Payroll Office at 3903 Brooklyn Ave. NE, Box
355655, Seattle, WA 98105-6694.
- Changing a direct deposit bank account requires two pay periods to take
effect (during that time you will receive a paycheck at your department on
payday).
- To stop a direct deposit, you must fill out a new Direct
Deposit Authorization form.
- Mail the form to the Payroll Office at 3903 Brooklyn Ave. NE, Box
355655, Seattle, WA 98105-6694.
- The automatic deposit will be canceled or stopped in one pay period.
- If you stop your direct deposit, you will receive a regular paycheck
at your department on payday.
