PERS 2 & 3 Enrollment

General Enrollment Information

Note: This page applies only to UW classified staff. If you are not in this group go back to the Retirement Plans web page and select the appropriate appointment type.

Newly-eligible employees must choose between PERS 2 and PERS 3 within the first 90 days of their PERS eligibility date. If you do not choose a plan by returning a properly completed form to the Benefits Office, the University is required by law to irrevocably enroll you in PERS 3, with a 5% payroll deduction.

Pre-Enrollment

Carefully review PERS Plan 2 and PERS Plan 3

PERS Enrollment

  1. Open the DRS Member Information form.
  2. Enter your choice of either PERS 2 or PERS 3.
  3. Complete all required information.
  4. Print, sign and send your enrollment form to UW Benefits Office, Box 359556.

PERS Survivor Benefit

There is a $150,000 survivor benefit for PERS members if death results from an employment-related injury. If you are a new PERS member, complete the Beneficiary Designation form.
