Enrollment Steps

If you are ready to enroll in the medical and dental insurance plans you have chosen, complete the following steps.

  1. Download the Medical/Dental Enrollment Form.
  2. Read all of the instructions before completing the form.
  3. Have the information on your worksheet available.
  4. If you are adding a qualified domestic partner, complete the Determining Dependent Tax Status Worksheet AND the Declaration of Tax Status Form.
  5. Complete and sign the form(s).
  6. Keep a copy for your records.
  7. Return the original form to the UW Benefits Office, Box 359556.

Verify Your Health Insurance Enrollment

Allow 8-10 working days before you attempt to verify your coverage. There are two ways to verify your insurance coverage on line:

  1. UW Employee Self-Service (ESS) - After you log in with your UW NetID, click the "Insurance" tab to view the insurance plans in which you are enrolled.
  2. Health Care Authority's website
    • Choose "Public Employee/Retiree"
    • Complete PEBB sign-in window
