Long Term Disability (LTD) insurance replaces a portion of your income if you are unable to work due to illness or injury. View the LTD website to learn about this important benefit.
Eligible employees are automatically enrolled in the basic LTD insurance plan at no cost. You may purchase additional, optional insurance for a monthly premium. Your premium rate is determined by both the waiting period you select and your retirement plan.
Newly-hired or newly-benefits-eligible employees who enroll in optional LTD coverage within the first 31 days of employment/eligibility are not required to provide health information or proof of insurability. After this 31 day window you may apply to enroll but evidence of insurability will be required. If the insurance company requests a medical examination it will be at your expense.
If you wish to enroll in the optional Long Term Disability insurance, complete the following steps.
If you enroll within the first 31 days of employment, coverage will be effective the first day of the next month. Otherwise, coverage will be effective the first day of the next month following review and approval by the insurance company.
UW Physicians (UWP) offers an LTD plan to its member doctors. Contact UW Physicians at 206-543-6420 for more information.
