Before your milestone anniversary date, your department's service recognition coordinator will give you a Service Award Selection Form. To select a gift, check the appropriate item on the form and return the form to your coordinator, who will process your selection.
Additional service recognition gift options include applying the value of the gift to one of the other recognition options such as Husky Dollars, UW athletic event tickets, a UW fund donation, and UW Performing Arts event tickets.
