The University of Washington maintains an official personnel file (OPF) for each employee in one of four Human Resources Operations offices. The official personnel file contains employment records required for business and legal purposes.

Employees have the right to access their official personnel files, submit documents for inclusion in the official personnel file, petition for the retention of materials in the personnel file that would otherwise be removed or destroyed. An employee may authorize another person to have access to the employee's official personnel file.

Accessing your Official Personnel File

Use the Official Personnel File Access Request Form to request access to your file . If you are unable to access this form via this web site, contact your HR Operations Unit to obtain a copy of the form.

HR Operations Unit Contact Information:

You will be contacted within three business days of receipt of the completed Official Personnel File Access Request Form to schedule an appointment to review your file. A member of the HR staff will remain with you while you review your file.

Obtaining Official Personnel File Photocopies

Photocopies of file contents will be provided without charge according to the following procedure:

  1. An employee may request one complete copy of the official personnel file once.
  2. After an initial copy is make only new documents placed in the file since the most recent photocopy request will be copied.
  3. Human Resources will provide a photocopy of OPF documents no more frequently than once every six months.
