Graduate appointees, fellows, and trainees in eligible positions can participate in the Graduate Appointee Insurance Program (GAIP), a program negotiated between the UW and United Auto Workers on a three-year basis.

The UW pays the monthly premium for student appointees' insurances listed in the next section.

Eligibility

To be eligible for GAIP benefits, you must be enrolled for at least 10 credits for the quarter, and your position must be 50% FTE or greater and paid on five of the six pay days of the coverage period. Your department—by entering information into the payroll system within a predetermined deadline—confirms that you will meet the eligibility requirements. Below is a chart of positions eligible for GAIP.

Eligible Graduate Appointees

The following table provides the titles and earning types which, in combination with the appointment and academic requirements, constitute an eligible position:

* Note: GSA is Graduate Student Appointment with benefits; GSO is Graduate Student Appointment with OASI benefits (i.e. reduced credit hours).

** Job Code 0854, Graduate Research Student Assistant (GRSA), is limited to appointments during the summer quarter only. This job code may not be used during fall, winter or spring quarters.

Eligible Fellows & Trainees with Stipends

The UW also pays the monthly premium for Fellows and Trainees in eligible positions. You must be enrolled for at least 10 credits per quarter, and your position must pay at least $800 per month, and you must be paid on at least five of the six paydays of the coverage period.

The following tables provide the titles and earning types, which in combination with the appointment and academic requirements constitute an eligible position:

*SWB refers to Stipend with Benefits. SWT is the equivalent of SWB for non-resident aliens.

Self-Pay Eligibility

The self-pay option allows you to participate in the GAIP benefits by enrolling and paying premiums directly to LifeWise Assurance Company (LifeWise). Review all self-pay information.

Dependent Eligibility

Students who are eligible and enrolled in this program may also enroll eligible dependents. The same dependent eligibility rules apply to all four quarters. An eligible dependent is defined as:

• Your legally married spouse
• Your qualified domestic partner if:
  • Both a Declaration of Marriage/Domestic Partner form and a Declaration of Tax Status form are submitted by the deadline for enrolling dependents. See GAIP forms.
• Your dependent children through age 25, including:
  • Your adopted children
  • Children legally placed for adoption, including a child for whom you've assumed total or partial legal obligation for support in anticipation of adoption (documentation required)
  • Stepchildren
  • Foster children or children for whom you're the legally designated guardian (documentation required)
  • In addition, your child will be eligible for coverage under this program if required by a court order and if a copy of the court order is provided
• Your dependent children who are incapable of self-support because of a physical handicap or developmental disability (documentation required; email the benefits@uw.edu for more information).

Enrolling Dependents Mid-Quarter

A dependent who becomes newly eligible as a result of a qualifying life event (such as marriage, birth, or adoption) may be added mid-quarter if:

• You are covered under this program, and
• You complete the appropriate enrollment form within 31 days of the date of your marriage or same-sex domestic partnership registration, or
• You complete the appropriate enrollment form within 60 days of the date of birth, placement for adoption, or date of adoption.

The steps to take after the event occurs are :

1. Enroll your dependent with the current plan administrator, LifeWise Assurance Company (LifeWise).
2. Complete and mail required enrollment forms to LifeWise. Forms can be found at: http://student.lifewiseac.com/uw/gaip/Forms.aspx
3. LifeWise will contact the UW to update your status.
4. If you need assistance, contact LifeWise Customer Service at 1-800-971-1491.

Summer Quarter

Graduate appointees, fellows, and trainees in eligible positions may participate in GAIP during summer quarter based on eligibility status. See Summer Quarter Information.
