Graduate appointees, fellows, and trainees in eligible positions can participate in the Graduate Appointee Insurance Program (GAIP), a program negotiated between the UW and United Auto Workers on a three-year basis.
The UW pays the monthly premium for student appointees' insurances listed in the next section.
To be eligible for GAIP benefits, you must be enrolled for at least 10 credits for the quarter, and your position must be 50% FTE or greater and paid on five of the six pay days of the coverage period. Your department—by entering information into the payroll system within a predetermined deadline—confirms that you will meet the eligibility requirements. Below is a chart of positions eligible for GAIP.
The following table provides the titles and earning types which, in combination with the appointment and academic requirements, constitute an eligible position:

** Job Code 0854, Graduate Research Student Assistant (GRSA), is limited to appointments during the summer quarter only. This job code may not be used during fall, winter or spring quarters.
The UW also pays the monthly premium for Fellows and Trainees in eligible positions. You must be enrolled for at least 10 credits per quarter, and your position must pay at least $800 per month, and you must be paid on at least five of the six paydays of the coverage period.

The self-pay option allows you to participate in the GAIP benefits by enrolling and paying premiums directly to LifeWise Assurance Company (LifeWise). Review all self-pay information.
Students who are eligible and enrolled in this program may also enroll eligible dependents. The same dependent eligibility rules apply to all four quarters. An eligible dependent is defined as:
