The UW Benefits Office manages the plan but does not handle claims or enrollment. Please contact LifeWise on issues related to claims, enrollment, and benefit provisions.

Non-Network Referral Service

The GAIP plan allows you and your enrolled, eligible dependents to seek medical treatment wherever you please. However, your cost when using a non-network provider is higher than if you see a network provider. Review the plan book and summaries for more information on the network providers and notice the differences in benefits.

Before seeking care, contact LifeWise to find a contracted provider in your location. If you use a contracted provider, you will not have to pay any amount over the allowable charge.
