Changing Medical/Dental Enrollment
for Faculty, Staff & Librarians

Special Open Enrollment—Change in Employment Status including Layoff

When health plan availability, benefits, or cost of insurance is affected by a change in employment status for you, your spouse, or Qualified Domestic Partner.

Options Actions
If you previously waived medical coverage, you can:
  • Enroll yourself in medical
  • Enroll yourself in medical to add eligible dependent(s) to medical and/or dental

Within 60 days of the event submit the following forms to the Benefits Office:

If you are already enrolled:
  • You can enroll your eligible dependent in medical and/or dental.
  • In the event of layoff from UW, you can change your medical and/or dental plan.
  • In the event of your spouse/QDP's job loss, you can change your medical and/or dental plan, provided you enroll the eligible dependent.

Within 60 days of the event submit the following forms to the Benefits Office:

Coverage Begins
  • Coverage and/or plan change begins on the first day of the month following the change in status. Unless the change in status occurs on the first day of the month, then coverage and/or plan change begin on that day.
